Release of Care

Registering your child for classes at CFT releases the care of your child(ren) to Centralia Faith Tabernacle from the time they are released into the class until you pick them up. Once they are picked up from their class they are released back into your care.

Procedures

Sign-in Procedures: One parent will drop off and pick up child at the classroom door, worker will fill out the sign-in book for attendance, make note of the parent who dropped off/picked up child, and time of pick up. Parents will assess child to ensure no symptoms before coming to class. Parents will send a snack and drink for their child, and will change diapers if needed.

Consent

Consent is given to participate in Children's Class or Nursery.

Consent is given to take pictures and videos of my child(ren) in accordance with the privacy policy.
